Hi, I’m Vanessa Patrick, the Post School Transition Officer here at Mitchelton Special School. I’m passionate about supporting students and their families as they prepare for life after school. My role is all about helping young people take the next step with confidence, whether that’s further education, employment, community involvement, or independent living.
Transitioning from school can feel overwhelming, but you’re not alone. I work closely with families, staff, and external services to develop tailored transition plans that reflect each student’s goals, strengths, and needs.
Here’s how I can help:
Planning and Coordination: I support students and families in planning what life looks like after school, this could include employment, study, or supported independent living. Together, we build a personalised pathway that fits each individual.
Employment & Work Experience Guidance: I support students in exploring job opportunities, training programs, and supported employment. I also help organise meaningful work experience placements in the community to build real-world skills and confidence. In addition, I assist students with job readiness skills such as writing resumes and understanding workplace expectations.
Connecting with Services: I help families access key supports such as the NDIS, community services, and government programs that can assist with a smooth transition into adulthood.
Family Support: I understand this journey can be a big step for families too. I’m here to guide you through the process, answer your questions, and connect you with resources that make the road ahead a little clearer.
My goal is to make sure every student feels prepared and supported for the future they want, and that families feel confident every step of the way.
I work in this position on Tuesdays and Wednesdays, but you’re welcome to reach out to me at any time. I’m always happy to chat and support you however I can.
